In light of the potential cuts, representatives have voiced concerns about the impact on relapse rates, the health care system, and overall health outcomes. Current and former employees of SAMHSA have expressed worries about the effect of layoffs and policy changes on projects aimed at addressing mental health and substance use disorders in marginalized communities.

During the Biden administration, SAMHSA experienced growth in budget and staff to address persistent underfunding, but the Trump administration’s proposed budget cuts could reverse this progress. Despite declining overdose fatalities since 2023, rates are still higher than in 2019, leading to concerns about reducing SAMHSA resources. The potential folding of SAMHSA into another health agency or reverting to 2019 staff levels could hinder oversight and accountability in addressing the ongoing addiction and mental health crises in the country.
